Down On The New Corner (in case you hadn't noticed)

One inch above the surface of the uneven pavers.
  Was this in the original plans?

Cracked concrete sidewalk
on the northeast corner

A cap of some sort would look nice and keep water from
accumulating in the hole, preventing the post from
 rusting.  It would also keep litter and cigarettes
from finding their way in. (northeast corner)

Either dead or soon to be.  How much water has
DPW fed this tree in the past week?  None?
